The UK media regulatory Ofcom is unhappy with ITV's use of Arma 2 footage in a documentary about the IRA having bought weapons from Libya. Viewers "were misled" by it.
The programme caused a "significant breach of audience trust," and Ofcom have ruled it "materially misleading," breaching Rule 2.2 of the Communications Act of 2003. Bad ITV, bad.

The episode was the first of ITV's "Exposure" series. The issue was that the footage was instantly recognized by ArmA II fans as a clip from a gameplay trailer for the PC first person shooter.
