If you do not already have a folder called
usermaps in your cod4 directory then you need
to create one.Then in that folder create a
folder called mp_doomed.In the mp_doomed folder
put the mp_doomed.ff and mp_doomed_load.ff
files.Then place the mp_doomed.iwd file in the
mod folder of your choice(ie which ever mod
you use).

To start the map,open console and type
map mp_doomed.
